Who is Eligible for Title V Programs?

CSHCN programs usually serve children (under age 18) and young adults from birth to age 21. Whether someone is eligible usually depends on the following:

  • Whether his or her specific diagnosis is included in the state’s program.

  • Physical or developmental needs caused by his or her disability.

  • Family income level.

A chronic, disabling condition such as juvenile arthritis may be included as a “special health care need” in your state’s CSHCN program. You will need to check with your particular state’s program to make sure. Also, to qualify, you will need to fall below a maximum income level set by your state.
