A Directory of State Title V CSCHN Programs published by the Institute for Child Health Policy is available online at: http://cshcnleaders.ichp.edu/TitleVDirectory/directory.htm. It was last updated in 2003, so some of the information may not be current.
Depending on where you live, you may find an online application form for your state. You should first learn about the services or assistance your state’s CSHCN program offers and then decide whether it will serve the child's needs.
