How Do I Apply for S-CHIP?
Most states will ask you to fill out and mail a 2-to-4 page application. In some states you can do this over the phone or on the Internet.
Most states have one application for both S-CHIP and Medicaid that will be used to determine which program you may qualify for.
Each state application is slightly different, but the types of information you have to provide may include:
- Family income
- When the child was last covered by health insurance
- How many people are in child’s family
- Cost of childcare
- Whether the family receives or is eligible for other benefits
- Child’s U.S. residency status
You can get help in filling out the application form by calling your state’s S-CHIP program.
To find out whether the child qualifies and how to apply call 877-KIDS-NOW (1-877-543-7669). This number allows you to directly reach your state program’s office. The call is free and confidential. Spanish-speaking operators are also available if you ask for them. Your state program’s written materials may also be available in a variety of languages. Program assistants may be able to tell you about people in your community who can help you complete the application.
On the Internet you may log onto http://www.insurekidsnow.gov and click on your state. Most states have online application forms.
In most states, you will have to re-apply for your child’s coverage every six or 12 months. Your state program should contact you in writing when you have to re-apply. They may send a form with pre-printed information that you have to verify or change.
After you submit your application, you may hear back in as little as five to 10 days, or as long as 45 days.
If it is determined that your child is eligible, coverage may start at the beginning of the next month or, possibly, the month after that.
Because of budget shortages, some states may have capped their enrollment, and new applicants may be put on a waiting list. This makes it especially important that you apply as early as possible.
