You apply for Medicaid by filling out an application. Depending on your state, this may be done through a paper application, through the Internet, or, in some states, by telephone. You may also have to meet with local social services personnel to apply.
Each state application is slightly different, but the types of general information you have to provide include:
Family income and other financial resources
How many people are in the child’s family
Whether the family receives or is eligible for other benefits
U.S. residency status of child and other applicants
