How Do I Apply for High-Risk Insurance?

You apply for a high risk insurance pool by filling out a form and providing the requested information and documents. The National Association of State Comprehensive Insurance Plans keeps a list of all the states that have high risk insurance pools. You can get a copy of this list online at: http://www.naschip.org/states_pools.htm.

You can also contact the National Association of State Comprehensive Health Insurance Plans at c/o Communicating for Agriculture & the Self-Employed, 8009 34th Avenue S., Suite 360, Bloomington, MN 55425, (952) 746-6612, Fax (952) 746-6611

You may also call your state’s department of insurance to ask whether a state high risk insurance pool is available and, if available, how to apply.

Each state will have its own application form that you will have to complete. Be sure to ask representatives of your state's program for its qualifying rules and how long it will take for an application to be considered.
