Benefits of Enrolling

We have included worksheets with the programs and laws we’ve described. Worksheets for the benefit programs include some of the main questions you will find in the state or federal application forms. Completing a program worksheet will give you a good head start because it will get you started on collecting the information you'll need when you fill out the official application or talk to program representatives. Worksheets for the laws described include some of the main questions you will be asked if you decide to use the benefits provided by a law on behalf of your child with juvenile arthritis or a related condition.

As an enrolled participant, you will be provided a user name and password that will allow the information you enter in these worksheets to be saved for you to return to later. This will allow you to return to finish a worksheet or to change information in it. The Web site is also helpful in not making you enter the same information twice. If two different programs ask for the same information, you only have to enter it in one program’s worksheet and it will automatically be put in the other program’s worksheet.

If you choose not to enroll as a participant, you will still have access to the worksheets. However, the information you enter will be saved for only ten days, after which it will automatically be deleted. You will always have complete access to the information about the Laws/Programs whether you enroll or not.

Additionally, as a participant you would be providing us with information for research on whether our Web site is helpful, how it may be improved, and the broad demographics of who is using it.

Whether you participate or not, you will be provided with a User Account name that you can use to access any information that you have input. You will want to write down the User Account Name or print the page that provides it and keep it in a secure place so you can use it later.
